Dr. Jerra Dooley Rate: $145.00 per *session

Brandon Mallette Rate: $125.00 per *session

Lori Barstow White Rate: $100.00 per *session

Matt Kirkpatrick: $75 per *session

     * Sessions are 45-50 minutes

Group Session – Vary depending on group

AD/HD Testing: $420.00 (1st session – 2hrs, 2nd session – 1hr)

Insurance

Fortify Christian Counseling, LLC does not bill your insurance directly. However, some of the counseling services provided by our Licensed Professional Counselors may be covered as part of your out-of-network insurance benefits. I’d recommend asking these questions to your insurance provider to help determine your benefits:

– Does my health insurance plan include mental health benefits as part of out-of-network benefits?

– Do I have a deductible? If so, what is it, and have I met it yet?

– Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

– Do I need written approval from my primary care physician for services to be covered?

– Do I need to submit anything other than a superbill from my mental health provider?

Cancellation Policy

If you cannot attend a scheduled session, please provide notice at least 48 hours in advance. Sessions canceled with less than 48 hours’ notice will be charged the full fee.
